Ending the HIV Epidemic in Houston

For the past two years, AFH has undertaken a strategic planning process to realign everything we do around the national plan to end the HIV epidemic. Reflected in our new mission statement, we seek to realize this goal through four strategies or "pillars."

Pillar 1

Diagnose all persons living with HIV as quickly as possible. This includes AFH's outreach, education, and prevention programs community-wide and inside Texas prisons; and HIV/STI testing in-office, online, and at-home.

Pillar 2

Treat persons living with HIV rapidly and effectively to reach sustained viral suppression.  While AFH does not provide primary health care, we do significantly impact the "social determinants of health." These are key drivers that impact health outcomes such as housing and food security, income and insurance, mental health and wellness, equitable access and patient navigation, community partnerships, and overcoming stigma and fear. Our client support services empower individuals living with HIV who are most vulnerable to being without care to access care, adhere to treatment, and, when possible, achieve sustained viral suppression. In 2021, AFH began the START (Strategic, Targeted, Anti-Retroviral Treatment) Now program for patients newly diagnosed with HIV or those who have been out of care.

Pillar 3

Prevent new HIV transmissions through proven methods, including STI treatment, pre-exposure prophylaxis (PrEP), Same-Day PrEP, nPEP, condom distribution, and syringe services program.   AFH endorses the message of U=U or Undetectable Equals Untransmittable. A person living with HIV who accesses and remains in treatment and achieves sustained viral suppression will be unable to transfer HIV through sex. 


This is key to helping people living with HIV lead healthy lives and prevent new HIV cases. Moreover, AFH offers PrEP in-office, online, and at home to ensure that this medication is available to all who need it. In 2020, AFH began partnering with the Houston Harm Reduction Alliance in collaborative outreach and advocacy for effective syringe service programs to reduce the risk of HIV transfer among persons who inject drugs.

Pillar 4

Respond quickly to potential HIV outbreaks with necessary prevention and treatment services for people who need them. Collaborating with the City of Houston Health Department and combining our shared resources and assets, AFH works closely with community leaders to identify areas of our communities that need additional presence, outreach, information, and testing. Our goal is to ensure accessibility to needed prevention and care services for all individuals vulnerable to HIV.


AFH is passionate about ending the HIV epidemic in the  Houston area. We support the nationally coordinated plan and comprehensive strategy at the state and local levels. Until we achieve our mission, AFH will be here supporting those most vulnerable in our communities and advocating to ensure that access to vital care is equitable, affordable, and stigma-free.


History of AFH

AIDS Foundation Houston, Inc. (AFH), founded in 1982, is a 501(c)(3) nonprofit corporation that was the first AIDS Service Organization in Texas and remains a national leader in HIV/STD programming. With the goal of creating a community where HIV is stigma-free and rare, and people have equitable access to care, AFH works towards the mission of ending the HIV epidemic in the Houston area. AFH services include prevention education, PrEP Care, nPEP, HIV Testing and Rapid Start Treatment, STI Testing and Treatment, outreach in Texas prisons, supportive housing, food assistance, a summer camp for children, and supportive services to all persons affected by or at risk for HIV. We are so excited to be celebrating 40 years or service to the greater Houston community.
